I, the instructor, agree to adhere to all American Heart Association (AHA) policies and practices as outlined in the Program Administration Manual (PAM). I commit to maintaining the highest standards of instruction, putting the safety and well-being of all students first in every class.

I understand that course completion cards are solely for my students and are not to be provided to other instructors or used for any purpose outside my own classes. I acknowledge that any violation of this agreement may result in the termination of my alignment with the Training Site.
